What is HomeSpace?
HomeSpace is a collaborative project that was developed to
support people with developmental disabilities and their families who
are homeless or being evicted. Agencies working together on this
program are Lane County Human Services Commission, St. Vincent de Paul
Society, Mainstream Housing Inc. and Lane County Developmental
Disabilities Services.
The mission of
HomeSpace is “supporting people with
developmental disabilities to maintain affordable housing, increase
independence and self-determination.”
HomeSpace offers housing with subsidized rent and on-going
support to work on issues that have caused prior homelessness. The goal
for every participant is to maintain safe, affordable housing, and to
enhance independence and self-determination. Participants must agree to
work with project staff in order to be involved in this program. Before
moving into an apartment, HomeSpace staff work with individuals to
develop a support plan that meets their specific needs and desires. The
plan is created with a person-centered approach to promote
self-determination. This support plan will then become part of the
lease agreement.
Eligibility
Requirements
Participants
in the HomeSpace program must meet the following criteria:

One individual in a household must have a developmental
disability as defined in the Oregon Administrative Rule.
·
The individual must be receiving case management through
Lane County or Full Access Brokerage.
·
Individuals must be homeless or about to become homeless
due to an eviction or other urgent circumstances.
Supports
HomeSpace
offers comprehensive supports to help ensure the participants are given
the best opportunities for success. Participants will work closely with
an Advocate from Mainstream Housing Inc. and an on-site Resident
Services Coordinator (RSC).
Role of
HomeSpace Advocate
·
Conduct regular home visits of all project participants.
·
Identify participant’s specific support needs.
·
Assist participants to access in-home and community
supports.
·
Work closely with the Property Managers and cooperating
housing agencies to ensure that participants are following policies and
procedures in order to successfully maintain their housing.
Role of
Resident Services Coordinator
·
Offer friendship and support.
·
Be on-site regularly and available to answer questions.
·
Arrange social events/activities for residents.
The Resident
Services Coordinator and HomeSpace advocate work together to ensure to
the extent possible for the success of each participant.
